Expectations of Membership
CCPM is a network of otherwise unaffiliated, not-for-profit healthcare organizations spanning the state of Maine. In order to be considered as a member organization, an organization must commit to compliance with several expectations of membership* outlined below.
Patient-Centered Medical Home Certification
Minimum of 1.0 FTE RN Care Manager
Established Quality & Care Management Programs
Data Reporting Capabilities
Read-Only EMR Access for CCPM Staff
Value-Based Contract Participation
Citizenship and Participation
Risk Adjustment and Code Capture Training
Value-Based Payment Readiness Assessment
*At the time of joining, or commitment to compliance within 6 months of joining CCPM. Organizations that are unable to satisfy the expectations of membership may be considered for an alternative or affiliate membership.
