A Joint Commitment to Address Food Insecurity in 2024
In 2023, CCPM’s Board of Managers decided to select an SDOH Area of Focus for 2024. The Board selected addressing food insecurity throughout the state with its Food as Medicine initiative. The initiative consists of ten operational commitments (outlined below), with an overarching goal of promoting food security in Maine in alignment with CCPM’s mission to ensure effective, high-quality care for its communities.

Identify and apply for grants to promote food security.

Screen patients for food insecurity

Establish a connection to culturally appropriate food sources

Assess members’ current practices related to screening and addressing food insecurity

Identify and share best practices, workflows and strategies to advance food security work

Incorporate progress and outcomes of the initiative in CCPM’s 2024 Annual Report

Develop a press release outlining key elements of the initiative to drive awareness

Join advocacy efforts to reduce barriers that hinder Mainers from achieving food security

Engage with key stakeholders to foster collaboration, share insights, avoid duplicating efforts and increase impact
10
Conduct network mapping activities to develop a comprehensive resource repository for food access resources
