Community Care Partnership of Maine (CCPM), an Accountable Care Organization comprised of seventeen Federally Qualified Health Centers (FQHCs) and four community hospitals throughout Maine, is pleased to announce the recent addition of Maine Mobile Health Program (MMHP) as its first ever affiliate member. Together, CCPM’s member organizations commit to improving the collective health of their communities through the coordination of sustainable, innovative, and effective patient-centered care.
“The Maine Mobile Health Program is excited to be able to join CCPM and participate in a collaborative effort to improve health care in Maine. We hope to be able to share our experience and knowledge in caring for a highly mobile and resilient patient community; and are eager to learn from the other CCPM partners in improving quality of care,” said Demetri Blanas, Medical Director of Maine Mobile Health Program.
Established in 2000, The Maine Mobile Health Program is a community health center and Maine’s only farmworker health organization, serving nearly 1,000 patients per year. MMHP provides mobile medical, behavioral health, and nursing care to patients at farmworker camps and other community settings across the state in furtherance of its mission is to improve the health status of Maine’s seasonal workers and their families by providing culturally appropriate care and services. By linking agricultural workers to care and resources, MMHP aims to reduce health disparities and inequities of access for people working in the agriculture industry.

As an affiliate member, MMHP will not participate in CCPM’s value-based contracts, but they will play an important role in CCPM’s learning community, participate in various quality improvement initiatives, and share best practices about delivering mobile care and integration of community health workers (CHWs) into care delivery. CCPM’s member organizations collectively contract with 12 payers and oversee more than 276,000 patient lives annually.
