Despite large variations in size, all 21 of CCPM’s member organizations have equal voting power on its Board of Managers. This egalitarian approach ensures the voices of all member organizations, regardless of size, are heard and honored in the decision-making process.
Customized Consultative Support
CCPM employs a team of experienced clinical leaders to partner with its member organizations on clinical operations and other facets of care delivery. CCPM’s staff work to be intimately familiar with the day-to-day operations of each site and function as trusted colleagues and workforce extenders for our member organizations.
Attribution-Agnostic Service Lines
CCPM intentionally designs all of its service lines, to benefit all patients served by its 21 organizations, regardless of their value-based contract attribution status. Our interventions and workflows are not payer-specific.
