The Theresa Fund is named in loving memory of Theresa Bray Knowles, who was the architect of the fund and served for many years as CCPM’s Chief Quality Officer. Theresa was a fierce advocate for serving the most vulnerable among us and was well-respected for her commitment to delivering high quality care. Theresa passed suddenly in 2021 following a battle with cancer. CCPM has worked to honor her legacy and work through The Theresa Fund and a number of other initiatives.

The Theresa Bray Knowles Gaps in Care Fund (“The Theresa Fund”) is designed to promote the overall health and wellbeing of our patients by providing items or services to those with a demonstrable financial need. Since its inception in 2019, the Theresa Fund has fulfilled thousands of requests, including home monitoring equipment for patients experiencing COVID-19.

Any member of the care team at our member organizations can submit a request to the fund via a short online form, which takes less than 5 minutes to complete. Most requests are approved and fulfilled within 2 business days.
