CCPM’s centralized Pre-Visit Planning Medical Assistants (MAs) are dedicated to identifying, organizing, and summarizing clinical data to produce comprehensive pre-visit plans for upcoming patient visits. The goals of our Pre-Visit Planning Service are intentionally aligned with the Institute for Healthcare Improvement’s Quadruple Aim framework.

Goals of the Service

  • CCPM’s Pre-Visit Planning Service alleviates administrative burden on healthcare providers and care teams by centralizing and organizing clinical data during the pre-visit planning phase.
  • Our work improves workflow efficiency, allowing more time for meaningful patient interactions (by both the MA and provider) and reducing stress associated with last-minute preparations.
  • Streamlined pre-visit plans to ensure that patients receive personalized, timely and efficient care during their appointments.
  • Reduced administrative burden enables more time for communication and relationship building between providers and patients.
  • CCPM’s MAs identify and outline gaps in care during the pre-visit planning process, ensuring that preventive screenings, immunizations, and other essential services are addressed at the time of the visit.
  • CCPM’s MAs recommend screening and referrals as indicated (e.g., for social determinants of health and care management), promoting early intervention and referrals to enhance care continuity and support overall population health.
  • CCPM MAs play an important role in minimizing unnecessary healthcare expenditures by identifying and addressing overdue care, preventive services, and screenings before they escalate into more expensive healthcare interventions.
