Penobscot Valley Hospital Joins Community Care Partnership of Maine

Community Care Partnership of Maine (CCPM), an Accountable Care Organization comprised of seventeen Federally Qualified Health Centers (FQHCs) and five community hospitals throughout Maine, is pleased to announce the recent addition of Penobscot Valley Hospital (PVH) to its membership. Together, CCPM’s member organizations are dedicated to improving the collective health of their communities through the coordination of sustainable, innovative, and effective patient-centered care.

It’s an opportunity to strengthen outcomes, support our dedicated staff, and enhance the overall health of our community through more coordinated, patient-centered care,” said Melissa Pelkey, Chief Executive Officer of Penobscot Valley Hospital.

Established in 1973, Penobscot Valley Hospital is a designated Critical Access Hospital that has served the greater Lincoln community for over 50 years. PVH offers a broad range of services, including 24/7 emergency services, inpatient care, laboratory and imaging services, surgical services, as well as primary and specialty are services.

With a mission to provide compassionate, high-quality care and improve access to vital services in the greater Lincoln area, PVH serves thousands of patients per year across three service locations in Lincoln, regardless of ability to pay.

CCPM’s member organizations collectively participate in 12 value-based payment arrangements, overseeing nearly 105,000 attributed lives.
