Community Care Partnership of Maine (CCPM), an Accountable Care Organization comprised of eighteen Federally Qualified Health Centers (FQHCs) and four community hospitals is pleased to announce the recent addition of HealthReach Community Health Centers to its membership. Together, CCPM’s member organizations are dedicated to improving the collective health of their communities through the coordination of sustainable, innovative, and effective patient-centered care.

Established in 1975, HealthReach provides comprehensive medical, dental, behavioral healthcare, and substance use disorder services. With a mission to promote, provide and coordinate quality healthcare for residents in Central and Western Maine communities, HealthReach Community Health Centers serves more than 26,000 patients per year across 12 service locations throughout Central and Western Maine.

Beginning in 2025, HealthReach will participate with Community Care Partnership of Maine in the Medicare Shared Savings Program (MSSP), a value-based payment model introduced by the Centers for Medicare & Medicaid Services (CMS) that aims to improve care quality while reducing healthcare costs.
